Instructions:
Before you begin
Do NOT eat, drink, smoke or chew gum 30 minutes before giving your saliva sampleStep 1: Spit into Tube
Attach the funnel to the tube and spit into it until the amount of liquid saliva (not bubbles) reaches the new fill line.
Step 2: Close the Lid
Hold the tube upright with one hand. Close the funnel lid with the other hand by firmly pushing the lid until you hear a loud click. The liquid in the lid will be released into the tube to mix with the saliva. Make sure that the lid is closed tightly.
Step 3: Cap the Tube
Hold the tube upright. Unscrew the funnel from the tube. Use the small cap to close the tube tightly. Discard the funnel.
Step 4: Shake
Shake the capped tube for 5 seconds.
Step 5: Fill Out Form
Fill out the Requisition Form, be sure to include: Name, date of birth, collected date, test requested and method of payment.
Step 6: Mail in Sample
Please send back the requisition form and the saliva kit with the unused label provided (stickers). Mail to Molecular Vision Laboratory at room temperature in the FedEx envelope provided:
